Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Please complete the Online Patient Registration Form prior to your first visit. Every detail you can offer Dr. David J. Harris regarding your symptoms will help him determine the best course of treatment. Very often, root canal therapy can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- Your referral slip and any X-rays if applicable.
- A list of medications you are presently taking, including any allergies.
- If you have dental insurance, please offer this information ahead of time so we can provide your estimated copayment amount. This will save time and allow us to help you process any claims.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.
Please discontinue the use of any pain medication at least six hours prior to your initial visit. Doing so will increase the accurate diagnosis of the proper tooth which needs treatment and help prevent successive visits back to us before actual treatment can occur. Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, latex allergy, etc.) or if you are on any medication (i.e. heart medications, osteoporosis therapy, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e antibiotics, for pre-med.)
If you prefer, you may complete your health history from home and securely submit it online before you arrive for your visit. This feature is optional as we have will have the same forms prepared if you prefer to read and sign just prior to your treatment instead. If your previous dentist has taken recent x-rays, they may be helpful to us, but are not necessary in completing your evaluation. The imaging technology we use is created specifically for the diagnosis of the specialty treatment your tooth requires. Throughout the course of your treatment, additional images will be necessary to give David J. Harris, DDS, MSD a visual reference that ultimately gives you the best final result.
